Adult Petition Generator (Non-Felon)

Below is an editable set of forms that are required by the Colorado Courts when you file your name change. This set of forms is for adults (generally over 18 years old, UNLESS you have active Colorado parenting orders) who do NOT have a felony conviction.

  • If you have a felony conviction or were ever adjudicated as a juvenile delinquent of an offense that would have been a felony if you were a minor, you must use the felony name change process.
  • If you are preparing these forms for a minor (under the age of 18, or 19 if Colorado parenting orders exist), you must use the minor name change forms.

These forms are offered “as-is” and your use of this automated generator DOES NOT constitute an attorney-client relationship with the Colorado Name Change Project or any of its Board/Staff/Volunteers.

After completing the below form, when you click SEND, you will receive an email at the email address you provide with an ODT file attached.  This ODT file contains your forms and can be opened in MS Word (paid), OpenOffice (free) or LibreOffice (free).

Tips:

  • Spelling is important, especially your new legal name.  If you misspell it and the judge grants your Petition, guess what:  your new legal name is…. whatever you wrote on your Petition.  Double and triple check spelling!
  • DO NOT SIGN the Petition at home.  It must be signed in front of either a notary or the Clerk of Court.
